Independent Living
The Independent Living program model is one in which a individual lives independently in their own home either by themselves or with a roommate(s). A home can range from an apartment to a privately owned home. In this setting, Summit Supports staff help individuals achieve unlimited possibilities, with a focus on fostering independence.
We offer support with daily living needs, health and safety, financial management, scheduling of appointments, prescription management and other customized services to fit each clients needs. Our goal is to help navigate a path that assures individuals can achieve their fullest potential in living a independent and fulfilling life.
Family Caregiver
The Family Caregiver program model supports our client living with a family member who provides residential services to them in their family home. This option provides individuals the choice to continue living at home where they are comfortable and with familiar caregivers.
Summit Supports provides quality services to the entire family. We ensure that not only does the individual receiving services have the supports they need to live an independent and healthy life, but that the entire family has a network of resources around them.
Host Home
The Host Home program model provides individuals with a stable and supportive private family home environment. Here they can participate in the everyday activities of family and community life.
The Host Home program begins with a highly personalized matching process and focuses on the personal needs and choices of our clients. We access our client’s interests, skills, and support needs to ensure the appropriate match that allows our clients to live life to the fullest in their community. Our goal is always to support in the development of a meaningful, long-term relationship between our clients and their Host Home Provider. That relationship should provide compassionate care, high quality of life, dedication, foster independence, and ensure the safety and wellbeing of those we support. Our proven model of placements leads to successful outcomes and powerful relationships that not only positively change the lives of those we serve, but the lives of our Host Home Providers.
Residential Settings
Ensuring that individuals are in the living setting that best fosters independence, self-advocacy, and success in all aspects of their lives is the goal of our Residential Team here at Summit Supports. We work directly with our clients, their families, caregivers, case manager, and other important relationships in our clients lives now and into the future. Person Centered supports means we thoroughly review the needs, desires, and interest of each individual to ensure their residential setting is a perfect fit. We strive to support individuals in achieving ultimate possibilities, with a focus on fostering independence.
